Goal-directed resuscitation in sepsis - a case-based approach
Program Description / Statement of Need:
The hemodynamic status of critically ill patients is complex often with complicating co-morbidities. As a result there are often conflicting therapeutic goals involved, such as maintaining hemodynamic instability and fluid management with ALI/ARDS. The challenges to managing septic shock are to find the right parameters to monitor and the end-points for adequate resuscitation. Current evidence supports the use of dynamic parameters rather than traditional static ones for fluid responsiveness. When caring for the complex patient, caveats with traditional assessment variables exist. Clinicians need to become more aware of new developments in managing septic shock. This session discusses current evidence for use of dynamic parameters in a goal directed approach to resolve sepsis, as well as caveats around other parameters used in goal directed resuscitation, such as ScvO2 and lactate levels.
Target Audience:
This program has been designed for a multidisciplinary physician and nurse audience including: ICU / Anesthesia / Peri-op / Intensivists / Trauma / Critical Care
Program Objectives:
By the end of the session the participant will be able to:
- Cite current research supporting the use of goal directed therapy.
- Define dynamic fluid responsiveness parameters.
- From a case scenario, determine if resuscitation end points have been met
Accreditation:
This CME activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of The Institute for Medical Studies (IMS) and ScientiaCME. IMS is accredited by the ACCME to provide CME for physicians. IMS designates this educational activity for a maximum of 1.0 hours AMA PRA Category 1 Credits™ . Physicians should only claim credit commensurate with the extent of their participation in the activity.
Nurses may claim credit for activities approved for AMA PRA Category 1 Credits™ in most states, for up to 50% of the nursing requirement for recertification. This course is designated for 0.75 AMA PRA Category 1 Credits™
